Wider Impact is pleased to have been commissioned by Black Country Partnership for Care (BCPC) to carry out an independent evaluation of the Bridging the Gap Project.

Project aims include developing the skills and resources to support person-centered transitions between social care and hospital health settings, with a focus on supporting a strategy of prevention and early interventions and re-ablement following hospital discharge.

Evaluation methodology will include (quantitative) desk research and (qualitative) field research, which will include one to one meetings, use of questionnaires and case studies.
